MI of antero-septal localisation
Sinus rhythm, rate around 100, Right axis deviation. Q waves in V2 - V4, ST elevations in V1 - V4.

Diagnosis, late presentation of Antero-septal Acute ST elevation MI
There is another finding in this ECG.
Sinus Rhythm at a rate of 100 bpm
Right Axis deviation at about -160
rS pattern in Lead I / V6
Qr pattern in lead V1
QS pattern with ST elevation in V2-V4

Diagnosis: Late presentation of anteroseptal AMI in a patient with RBBB ( right bundle branch block)
This patient has LPHB also.
sinus rhythm , sever right axiS deveation , qs complex in v1-3 with elevation of ST segment , poor progression of r wave in chest lead probably acute anteroseptal MI with hemiblock though the ST shape is not typical of infartion other possibility RVH, or ASD .,
, emphysema some times give us qs complex in v1-3 leads with partial RBBB ?? probably medical history and clinical notes of great help ,
acute anterior mi .rt axis deviation .posterior hemiblock .sinus tachycardia
