the EKG is suggestive of Ventricular tachycardia with heart rate of 200 bpm.

features suggestive of VT here are:
1. RBBB pattern with NORTHWEST AXIS (suggests origin from left ventricle)
2. FUSION BEATS
3. ATRIO-VENTRICULAR DISSOCIATION

my diagnosis would be an LV SEPTAL VT or FASCICULAR VT.
