this young unlucky lady has large muscular VSD and was diagnosed in childhood , and the family misunderstand the problem regard her condition as congenital or somthing natural and no need for treatment until later she was presented with cyanosis tiredness ,the image shows reversed shunt right to left flow through the VSD
