A 65-year-old male presents with increasing breathlessness and weight gain over the last three months.
He has been treated for heart failure by his GP, receiving furosemide and lisinopril.
On examination he has a pulse of 88 beats per minute regular and a blood pressure of 120/80 mmHg and his JVP is measured at 4 cm above the manubrio-sernal angle. Auscultation of the heart reveals an added diastolic sound. His chest examination is clear.
His abdomen is slightly swollen and he has shifting dullness to percussion. He has leg oedema to the knees. His lateral chest x ray is shown.
What is the diagnosis?

The right sided failure, ascites and pericardial calcification on x ray suggest a diagnosis of constrictive pericarditis.......thanks
