I understand thais is a single lead holter. Polymorphic VT coupled with a history of syncope associated with HOCM would be an indication of ICD..... Syncope and NSVT are two "major risk factors" for sudden cardiac death in HOCM.

as regard ECG its not apparent here i can only count HR 50pbm. anterior leads show severe ischemia

Its very difficult to have 2 acute thrombosis at the same time in LAD and RCA but possibly this ecg might be due to suboccluded LAD for long time with presence of collaterals between LAD and RCA (mostly the anastomosis in interventricular septum) so…

what is your source dr ashok? we traditionally view ghee as being harmful while olive and sunflower oil are considerd good. olive oil is actually recommended by ACC/AHA, although it ranks worse than ghee according to your data...

A 48 yr old male smoker developed acute onset chest pain. ECG no 1 is at presentation and no 2 within 15 minutes. he was thrombolysed with tenecteplase. we inserted a snare through the femoral route and hooked the guidewire from the jugular vein. the wire was snared and pulled out thereafter. the patient is doing great.
