early after open heart surgery(cabg:complex coronary intervention),In ICU : hypotension,sinus tachycardia ,jugular vein distension and anemia,urgent TEE was done and shows large localized pericardial effusion(hematoma) compressing right atrium and right ventricle(tamponade) with multiple fibrin stands inside.
